Orthodontics for teenagers- Level 3
The vast majority of patients seeking orthodontic treatment are teenagers. Their facial skeleton is growing and developing and this process plays an important role on the occlusion in all three planes of space. The aim of this module is for you to learn more about the growth and development of the craniofacial area and its significance in the treatment planning, execution of the treatment and achievement of a stable and predictable end result. You will be introduced to some of the fundamental orthodontic appliances that are currently being utilized in the daily clinical practice. We will pay a special attention to the treatment of Class II malocclusions, because they are the most common. We will discuss the orthopaedic corrections and the camouflage approach. You will learn when and how to use space maintainers in the mixed dentition and by using them you could reduce the need for extractions of permanent teeth later on.
Day 1:
08.00h. – 10.00h. – Theoretical session – Growth and development of the craniofacial area. Treatment of CL II, CL III, open bite, deep bite.

10.15h. – 11.00h. – Practical session – Clinical procedures required for the manufacture of lingual, palatal arch, rapid maxillary expander, quadhelih, twin block.
11.00h. – 12.00h. – Theoretical session – Application of the above mentioned appliances.

Orthodontic treatment of actively growing patients includes those in mixed and permanent dentition.
Many teenagers consider dental braces a piece of jewellery and would not mind wearing them to get a more beautiful smile. The dentist can use fixed metal braces, ceramic braces, or clear aligners to straighten misaligned teeth.
We normally initiate treatments when all permanent teeth have erupted. However, there are some indications for intervention in the mixed dentition years. The main goal of this module is to obtain a significant amount of practical and theoretical skills for dealing with teenagers.
What is the Level 3 course – Orthodontic treatment for teenagers?
Before we initiate traditional fixed metal braces treatment, the teenager should be free of tooth decay and capable of keeping an acceptable standard of oral hygiene.
We dentists are concerned with overall oral health, and we know that gum disease and white spot lesions will occur. We do not want to have these complications during the private orthodontic treatment of our young patients.
What are the chief concerns that bring them to our clinic looking for braces options?

Protruding teeth (especially upper incisors) and crowded teeth are usually the reason for the initial braces consultation. However, children, especially teenagers, can be extremely self-conscious and may have different bite issues or dental concerns that bother them. Various appliances can be implemented into the prospective treatment plan.
However, we aim for the best solution in a reasonable timeframe without overcomplicating the teeth straightening procedures. The rule KISS is equally important when we want to solve orthodontic problems during our teenage years.
The ideal treatment plan usually involves improving the appearance of the front teeth while creating a healthy smile and better bite. Facial aesthetics can also be improved, for example, when we reduce an increased overjet that causes a trapped lower lip.
Let’s consider for a moment the possible treatment options for a Class II Div 1 case. Suppose we have to treat an actively growing patient. In that case, we want to know if there is a skeletal discrepancy, proclination of the upper incisors due to a bad habit, or a combination of skeletal and dental deviations from the norm.
If the skeletal Class II is on a maxillary background, an extraoral appliance such as headgear, in combination with fixed braces, can be a valuable treatment option. However, wearing headgear requires a lot of motivation and discipline, which some teens might find an unacceptable treatment option.
Another valuable option is using a functional appliance to initially correct the overjet and fine-tune the occlusion with traditional braces afterwards. We prefer to use a Twin Block appliance when we are dealing with Class II on a mandibular background.
It allows us to address not only the sagittal but also the transverse dimension if necessary. In addition, we can retrocline the upper incisors, procline the lower ones, and control the eruption of the molars all at the same time.
Orthodontic appliances should be selected based on the case’s specific needs and the child’s preference. Whenever I treat a teenager, I make sure that the patient understands that my goal is not only to provide him with a beautiful new smile but also to obtain the best occlusion possible.
I personally feel that obtaining a nice static and dynamic occlusion at the end of orthodontic treatment will be the best service to the patient.
Unfortunately, due to our dependence on their cooperation, we are not able to get a perfect end result when we have to deal with an unmotivated teen. People who skip appointments and regular checkups usually do not have the best oral hygiene either. Gum problems such as inflammation, bleeding and even recessions could appear, requiring specific periodontal treatment.
